Someone films themselves running, sees their heel touch down first, and arrives at my door convinced they have found the cause of everything.
They then spend six weeks trying to land on their forefoot, develop calf and Achilles pain they did not have before, and run worse.
I have watched this happen enough times to be blunt about it. The heel is almost never the problem, and trying to fix it directly is one of the more reliable ways to injure yourself.
It is not which part of your foot lands. It is how far in front of you it lands.
What the Evidence Actually Says
Something like three quarters of recreational distance runners land heel first, and a substantial proportion of elite marathon runners do too. If heel striking were straightforwardly harmful, that would be very hard to explain.
The research comparing foot strike patterns has not produced a clean answer that one is safer. What it has produced is a clearer picture that different patterns move injury risk around rather than removing it. Forefoot runners get fewer knee problems and more Achilles and calf problems. Heel strikers get the reverse.
You are not eliminating load by changing where your foot lands. You are relocating it.
The Thing That Actually Matters
Where the foot contacts the ground relative to your centre of mass.
If the foot lands well in front of your hip, you have created a braking force, the knee is relatively straight at contact, and shock passes up through the joint rather than being absorbed by muscle. That is overstriding, and it is genuinely associated with injury.
Here is the connection people miss. Severe overstriding tends to produce a pronounced heel-first landing, because reaching forward with the leg naturally presents the heel. So heel striking is often a symptom of overstriding, and people see the symptom and treat that instead of the cause.
Fix the overstride and the foot strike usually sorts itself out. Fix the foot strike directly and you have changed a landing while keeping the reach, which is the worst of both.
Why Direct Cueing Backfires
Tell somebody to land on their forefoot and they do it by reaching and pointing the toe, which lands them just as far in front but now with the calf absorbing everything.
They have kept the braking force, added enormous load to the Achilles, and increased the metabolic cost of running because the calf is now working through a longer range on every stride. That is the recipe for the Achilles problem that shows up in week five.
The indirect route works better. Increase cadence by five to ten percent and the stride shortens automatically, the foot lands closer to underneath you, the knee is more flexed at contact, and the strike pattern drifts toward midfoot on its own without anybody thinking about their feet.
Same outcome. No conscious change to the landing. Far lower injury risk during the transition.
When I Do Care About the Foot
Rarely, and always as one input among several.
A runner with persistent knee pain who overstrides heavily is worth working with, but I will work on cadence and hip strength rather than on their heel.
A runner with recurrent calf or Achilles problems who has recently moved toward a forefoot landing, often after buying minimalist shoes, is worth moving back. That is a case where the strike pattern genuinely is the cause, because it was an artificial change.
And a runner who lands with an audible slap has poor eccentric control at the ankle, which is a strength issue rather than a technique one.
What to Do Instead of Worrying About It
Film yourself from the side, at the end of a long run rather than fresh, and look at one thing: how far in front of your hip is your foot when it touches down. That is the number that matters.
If it is a long way, raise your cadence by five percent, work on your hip and glute strength, and leave your feet alone entirely.
If your foot lands close to underneath you and you are a heel striker, you are fine. Genuinely, entirely fine, and you should stop reading articles about it.
The Bottom Line
Heel striking is not a fault. Overstriding is, and it often produces heel striking, which is why the two get confused.
Do not try to change where your foot lands. Change your cadence, strengthen your hips, and let the foot arrive wherever it arrives once it is landing underneath you.
